Please take this form to your GP for him/her to fill out when acquiring your Mental Health Treatment Plan. Having a Mental Health Treatment Plan makes you eligible for a Medicare rebate.
If you are a GP,
Please download and fill in this form. email us your referral letter together with your patient’s Mental Health Treatment Plan to refer@balancepsych.com.au With all the information clearly specified, it will save valuable time and minimize your workload.
